NC REACH: Creating a Culture of Wellness in Rural HIV Care

Abstract

NC REACH is a demonstration project funded by HRSA's Special Project of National Significance Initiative and seeks to build a medical home for people with HIV who are also homeless or unstably housed and who have a co-occurring mental health and/or substance abuse. This presentation will demonstrate the practical lessons learned to provide effective teamwork to increase retention on a research study.

Proposal Summary

The NC REACH team is funded by HRSA's SPNS initiative. Our participants have an array of needs. However, the team's effort and effectiveness has been instrumental in keeping the patients engaged, retained, and linked in care. This presentation will convey the social determinants at play for this population, and disseminate knowledge about the distinctive challenges faced by clients in rural North Carolina and strategies for addressing them. Furthermore, we will discuss the innovative medical home model in a rural healthcare setting and how it has fostered a culture of wellness and facilitated engagement and retention in care. The panel of NCREACH team members will discuss this unique experience and insight and share best practice and evidence based practices. Team members will convey how to overcome the challenges in providing care to this vulnerable population. Moreover, the team will specifically discuss how the services provided by their clinical team, case managers, service coordinators, and network navigators has engaged, retained and linked patients into care.

Relevance And Significance

This information would be very beneficial to other research studies regarding retention and enhancing the quality of care for our patients. This information will particularly be of interested to other rural healthcare agencies who are conducting research studies. We hope to discuss our lessons learned to promote research for a culture of learning.These presentations will give participants an opportunity to learn more about the best practice and evidence based interventions conducted at CommWell Health.
