Abstract or Description

Partnerships engaging the community is a powerful tool in addressing health disparities in both rural and urban communities. The first presenter will discuss a series of health assessments used to inform a future community health center in one of the oldest and formerly operating African American schools in rural Georgia. The second presenter will discuss how they expanded an urban elementary school’s school-based behavioral health center model to address primary healthcare needs to eliminate barriers and improve access to services for a community with significant healthcare disparities. This session will highlight how two partnerships identified health needs and assets to move towards building safer and healthier communities.
