Mom’s Heart Matters: A Comprehensive Approach to Support Mothers and their Circle of Care

Abstract

Background: Hypertensive disorders of pregnancy and other cardiovascular conditions are among the leading causes of death for women within one year of pregnancy. Traditional care models may not provide the infrastructure for the healthcare team to address multi-dimensional concerns impacting patients, resulting in missed opportunities to prevent adverse outcomes. Mom’s Heart Matters (MHM) is a comprehensive postpartum support intervention designed to reduce the alarming maternal morbidity and mortality rates, with a focus on addressing the disproportionate impact on women of color in rural communities.

Methods: MHM employs an interactive remote digital platform based in behavioral and cognitive science designed to improve perinatal wellness and reduce the rate of hypertensive and cardiovascular complications in the postpartum period. Patients are enrolled prior to discharge and receive a remote blood pressure monitor and mobile application. For the postpartum year, participants receive bi-directional, real-time support with breastfeeding, cardiovascular health, medication management, perinatal mental health, parenting, and social drivers of health.

Results: Since its inception in February 2023, MHM has enrolled over 172 mothers. Out of the 172 mothers, 40 required remote blood pressure monitoring and 17 high-risk patients required immediate intervention within the first week of discharge. The program has a 95% retention rate. 100% of participants identified as having signs of adverse events through the real-time monitoring, had no negative outcomes.

Conclusions: MHM is a transformative approach addressing a critical public health issue. By leveraging innovative technology to facilitate comprehensive patient-driven care, this platform has shown promising results in improving maternal health outcomes. Continued investment and expansion of this responsive infrastructure is paramount to save lives and strengthen the circle of care for mothers and babies. By combining remote monitoring, personalized support, and timely intervention, MHM is demonstrating a positive impact on maternal health in the community served by Liberty Regional Medical Center.
