Abstract

Discuss how health districts can leverage or develop a data-driven communtiy health assessment (CHA) and communtiy health improvement plan (CHIP) with multi-sector collaboration and community engagement through the Mobilizing for Action through Planning and Partnerships (MAPP) process to improve the health of citizens throughout Georgia and achieve health equity. This learning session will provide details on how to develop a CHA that tells the story of a community’s health and wellbeing through quantitative and qualitative data collected through three assessment tools: Community Partners Assessment, Community Status Assessment, and Community Context Assessment. Additionally, this session will provide information on how a CHA is used to develop a CHIP that mobilizes partners and defines health priorities and effective strategies. Specifically, this session will focus on how local County Boards of Health can participate in the development and implementation of a CHA and CHIP by promoting policy, systems, and environmental changes, aligning county strategic plans and community resources to the shared goals of the CHIP, and engaging their circles of influence.
