Abstract

Albany Area Primary Health Care strives to provide care that is affordable and accessible in the community that we serve. Our mission is to provide quality health care to everyone, and our core values are to always show compassion, always connect with our community, and continuously strive to improve. One of the various ways that AAPHC has upheld its core values is by creating an integrated oral health and primary care system that provides the community with more accessible and affordable care. My presentation will explain why and how Albany Area Primary Health Care has integrated oral health into primary health care. It will detail AAPHC’s journey with the creation of our first dental clinics, how we integrated our dental clinics into primary health care, and how we have created a sustainable integrated system. I will also provide details on methods of data collection, workflows that we utilize to create streamlined processes and efficiency, and how we have maintained sustainable integrated services.
