Abstract

Methods: Using the Public Health Accreditation Board (PHAB) Initial and Reaccreditation Standards and Measures, District 4 Public Health (D4PH) has integrated data-driven performance management, quality improvement, and planning practices into program and service processes and procedures.

Results/Anticipated Results:

  • Community Health Assessments: D4PH has established a community health assessment (CHA) for its 12 counites to provide information, resources, and data on the health factors and outcomes of the community. CHAs were used to develop a strategic plan and community health improvement plan and have supported D4PH and partners in data needs and “telling the story” of our communities for grant applications.
  • Program Performance Management and Evaluation: D4PH programs established goals, objectives, and performance measures that are tracked, monitored, and reported quarterly to staff and board of health members. Annually, programs review performance data to evaluate programs and identify best practices/opportunities for improvement and use the data to aid in program planning/decision making.
  • Quality Improvement: Data collected and analyzed helps D4PH identify opportunities for improvement and supports the implementation of formal quality improvement projects.

Conclusion: Data-driven approach to public health practice tells the story of public health successes and helps identify opportunities to continuously improve. Data is the necessary evidence to make informed decisions, so public health can best serve individuals to ensure optimal health for all.
